Responsibilities<\/p>\n
Coordinate and provide care that is safe, timely, effective, efficient, equitable, and client centred.
\nHandle case assignments, draft service plans, review case progress and determine case closure.
\nHelp clients achieve wellness and autonomy.
\nFacilitate multiple care aspects (case coordination, information sharing, etc).
\nHelp patients make informed decisions by acting as their advocate regarding their clinical status and treatment options.
\nDevelop effective working relations and cooperate with medical team throughout the entire case management process.
\nTake the extra mile and interact with patients to keep track of their progress and to ensure satisfaction.
\nRecord cases information, complete accurately all necessary forms and produce statistical reports.
\nPromote quality and cost-effective interventions and outcomes.
\nAssess and address motivational and psychosocial issues.
\nAdhere to professional standards as outlined by protocols, rules, and regulations.
\nJob Requirements & Skills<\/p>\n
Candidates should possess a B.Sc Degree in a related Healthcare field with 3 \u2013 6 years relevant work experience.
\nCompassionate with teamwork skills
\nExperience working with an EMR would be an advantage.
\nExperience with Ms office required.
\nEffective communication skills.
\nExcellent organisational and time management skills.
\nProblem solving skills and ability to multi-task.
\nSalary Budget
\nN200,000 (Monthly Net).<\/p>\n
